Speech Language Pathologist
Job Summary:
We are looking for a Speech-Language Pathologist (SLP) to provide comprehensive speech therapy services to residents in our Skilled Nursing Facility.
The ideal candidate will be responsible for evaluating, diagnosing, and treating speech, language, communication, and swallowing disorders in a diverse population of patients, ranging from post-acute rehabilitation to long-term care residents.

Key Responsibilities:
* Patient Evaluation: Conduct thorough assessments and evaluations of patients' speech, language, cognitive-communication, and swallowing abilities to identify treatment needs.
* Treatment Planning: Develop individualized treatment plans based on patient evaluations, goals, and physician recommendations.
Adjust plans as necessary to ensure optimal outcomes.
* Therapy Implementation: Provide direct therapy services to residents, including speech, language, cognitive-communication, and swallowing interventions.
Utilize a variety of therapeutic techniques and approaches based on patient needs and goals.
* Collaboration: Work closely with other healthcare professionals, including nursing, physicians, and occupational therapists, to ensure a holistic and coordinated approach to patient care.
* Family Education: Educate residents' families and caregivers on speech and swallowing disorders, as well as strategies to support communication and feeding needs at home.
* Documentation: Maintain accurate and timely documentation of patient evaluations, progress notes, treatment plans, and discharge summaries in compliance with facility and regulatory requirements.
* Compliance & Quality Assurance: Ensure that all services provided meet regulatory requirements and adhere to facility policies and standards of practice.
* Discharge Planning: Participate in discharge planning, providing recommendations for continued care and follow-up therapy services when necessary.
* Continuing Education: Stay current with the latest research, therapy techniques, and treatment modalities in speech-language pathology.
Participate in ongoing professional development activities.
Qualifications:
* Education: Master's degree in Speech-Language Pathology from an accredited program.
* Licensure: Current state licensure as a Speech-Language Pathologist in Oregon or eligibility for licensure.
* Certification: ASHA (American Speech-Language-Hearing Association) Certification of Clinical Competence (CCC-SLP) preferred.
* Experience: Previous experience working in a Skilled Nursing Facility (SNF) or similar healthcare setting is preferred, but new graduates are welcome to apply.
