Responsibilities:


* Provide direct patient care under the supervision of a licensed Occupational Therapist (OT)


* Implement individualized treatment plans as prescribed by physicians and OTs


* Complete daily documentation and weekly progress reports in compliance with state and payer guidelines


* Communicate patient progress and concerns with therapy and nursing teams


* Assist with caregiver and family education for safe discharge planning


* Participate in interdisciplinary team meetings and care planning


* Support restorative nursing programs and train restorative aides


* Maintain a clean, safe, and organized therapy environment


* Report equipment needs and safety concerns promptly


* Participate in ongoing training, in-services, and staff development

Qualifications:


* Active COTA license in good standing (required)


* Knowledge of the Occupational Therapy Practice Act


* Experience in skilled nursing or post-acute care preferred


* Familiarity with Casamba and PointClickCare preferred


* Strong documentation, communication, and interpersonal skills


* Team-oriented with a patient-centered approach
