Position Overview:
We are seeking a detail-oriented and experienced MDS LVN or RN to join our dynamic team.

In this role, you will be responsible for managing the Minimum Data Set (MDS) process, ensuring compliance with state and federal regulations, and collaborating with interdisciplinary teams to optimize resident care plans.

Key Responsibilities:


* Complete and submit accurate MDS assessments in a timely manner


* Coordinate care planning with clinical teams


* Maintain compliance with all regulatory requirements


* Support quality improvement initiatives

Qualifications:


* Current LVN or RN license in California


* Previous MDS experience preferred


* Strong organizational and communication skills


* Ability to work collaboratively in a fast-paced environment
