What You'll Do:


* Assess residents' physical conditions and develop individualized treatment plans


* Provide therapy sessions focused on improving mobility, strength, and function


* Collaborate with the interdisciplinary care team to ensure holistic resident care


* Educate residents and families on exercises and recovery plans


* Maintain accurate and timely documentation

What We're Looking For:


* Active Physical Therapist (PT) license


* Strong clinical skills and a passion for resident-centered care


* Excellent communication and teamwork abilities


* Experience in a post-acute or skilled nursing setting (preferred but not required)
