Case Manager - IMPACT

Lewis & Clark Behavioral Health Services (LCBHS) seeks a highly motivated case manager for our Individualized and Mobile Programs and Assertive Community Treatment (Impact) Team.

Team members provide intensive, wrap-around mental health support in the community for individuals with severe mental illnesses, focusing on home visits, medication management, therapy, life skills training (housing, employment, finances), and crisis support, aiming to reduce hospitalization and improve independence through a multidisciplinary, recovery-focused approach.

This is a daytime position with some weekend coverage required.

Key Responsibilities Direct Client Care: Conduct home visits and provide community-based support.

Therapeutic Support: Provide individual and group skills training, crisis intervention, and substance abuse support.

Life Skills Training: Assist with daily living skills, housekeeping, cooking, shopping, and transportation.

Vocational support: Assist with vocational training, job placement and support.

Case Management: Help with housing, finances (bill paying), employment, and accessing community resources.

Team Collaboration: Work closely with psychiatrists, psychologists, nurses, and other professionals.

Documentation: Complete assessments, treatment plans, and progress notes.
