Lone Tree Post Acute is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team on a part-time basis.

Position Highlights

Responsibilities


* Evaluate residents for speech, language, cognitive, and swallowing disorders


* Develop and implement individualized treatment plans


* Provide skilled speech-language therapy based on residents' needs


* Monitor progress and adjust treatment plans as appropriate


* Collaborate with nursing, therapy, and interdisciplinary team members


* Educate residents, families, and caregivers regarding treatment recommendations


* Maintain accurate and timely clinical documentation

Qualifications


* Current California Speech-Language Pathologist (SLP) license


* Master's degree in Speech-Language Pathology from an accredited program


* ASHA certification (CCC-SLP) preferred


* Experience in a skilled nursing or post-acute setting is a plus


* New graduates are encouraged to apply


* Strong communication, organization, and clinical skills
