Victorian Post Acute is a 90-bed skilled nursing and rehabilitation facility located at 2121 Pine Street in San Francisco with ample parking.

We are looking for a compassionate and dependable Social Services Assistant to join our team and help residents and their families navigate their healthcare journey with dignity, compassion, and support.

What You'll Do

As a Social Services Assistant, you will play an important role in supporting our residents and families throughout their stay.

Your responsibilities will include:


* Build meaningful relationships with residents and families and provide compassionate support.


* Assist residents with social, emotional, and psychosocial needs.


* Support care planning and help identify goals that promote independence, dignity, and quality of life.


* Provide emotional support to residents and families dealing with illness, disability, adjustment, grief, and loss.


* Assist with admissions, transfers, and discharges .


* Participate in discharge planning and connect residents with appropriate community resources.


* Interview residents, guardians, and sponsors to gather required admission information.


* Prepare admission packets, including resident rights, privacy notices, and admission agreements.


* Maintain accurate resident records and identification information.


* Maintain accurate bed availability records.


* Work closely with nursing, rehabilitation, administration, and other interdisciplinary team members.


* Maintain confidentiality and follow facility policies and applicable privacy requirements.

Requirements:


* High school diploma or equivalent required.


* Previous healthcare, skilled nursing, senior living, or social services experience preferred.


* PointClickCare (PCC) experience is a plus, but not required.


* Excellent communication and interpersonal skills.


* Strong organizational and documentation skills.


* Ability to manage multiple priorities in a fast-paced environment.


* Ability to work effectively with residents, families, staff, and community resources.


* Empathetic, patient, professional, and resident-focused approach.
