Health System Navigator

1.

Participant navigation and access support

• Support participants where progression is affected by system, access, communication, service or environmental barriers.

• Help participants understand available local options and make informed choices about support routes.

• Facilitate warm introductions, supported self-referrals and practical preparation for service contact through community partnerships & local authority.

• Track whether contact, attendance and engagement occur and agree alternative routes where needed.

2.

Local partnership development

• Build and maintain relationships with NHS, primary care, community health, local authority, social prescribing, VCSE, disability, skills and community partners.

• Maintain an accurate local service and community asset map, including eligibility, access routes, waiting times and accessibility options.

• Develop warm-referral arrangements and reciprocal communication routes with local partners.

• Attend local partnership forums and contribute to coordinated support around Confident Futures participants.

3.

Adviser and coach support

• Provide factual advice to EAs and Health Confidence Coaches about local services, referral routes and access barriers.

• Contribute to Health Confidence Case Reviews where system barriers are delaying progress.

• Help identify alternatives where services are delayed, unavailable, unsuitable or declined.

• Support teams to understand local gaps, pathway changes and community opportunities.

4.

Long-term condition, disability and accessibility navigation

• Use enhanced knowledge of long-term and fluctuating conditions, disability, neurodiversity and mental wellbeing barriers to understand functional and participation impacts.

• Identify environmental and accessibility barriers such as transport, digital exclusion, communication needs, appointment format, confidence and local provision gaps.

• Support reasonable adjustment conversations and practical access planning without providing clinical advice.

5.

Safeguarding and escalation support

• Recognise, respond to and escalate safeguarding, crisis or urgent concerns in line with Maximus policy and local pathways.

• Maintain accurate documentation of concerns, information shared, actions taken and outcomes.

• Operate with Safeguarding Adults Level 3 training and access to role-specific safeguarding supervision.

• Liaise with the Safeguarding Lead and Clinical Governance Lead where concerns exceed role scope.

6.

Continuous improvement and reporting

• Identify recurring access problems, referral gaps, waiting-list issues and local service capacity barriers.

• Monitor referral outcomes and support evidence for performance, participant experience and quarterly reporting.

• Feed local intelligence into continuous improvement, partnership development and solution design.

• Support quality assurance through accurate, timely and auditable records.
