LVN - Part Time

Licensed Vocational Nurse (LVN)

Concord Post Acute is seeking a compassionate, dependable, and skilled Part-Time Licensed Vocational Nurse (LVN) to join our dedicated nursing team.

The LVN will provide high-quality direct patient care, support clinical staff, and help ensure positive outcomes for our residents in a fast-paced post-acute care environment.

This position is ideal for an individual who is committed to delivering exceptional, resident-centered care while working collaboratively with an interdisciplinary team.

Position Responsibilities


* Provide direct nursing care in accordance with established care plans and nursing standards


* Administer medications and treatments as prescribed by physicians


* Monitor, assess, and document resident conditions, reporting changes promptly to the Registered Nurse or physician


* Assist with resident admissions, discharges, and transfers


* Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure continuity of care


* Maintain accurate and timely clinical documentation in compliance with state and federal regulations


* Promote a safe, clean, and supportive environment for residents, visitors, and staff


* Deliver compassionate, respectful care while maintaining resident dignity and independence

Qualifications


* Current, active California Licensed Vocational Nurse (LVN) license


* Current CPR/BLS certification


* Minimum of one (1) year of experience in a skilled nursing or post-acute care setting preferred


* Strong clinical, organizational, and assessment skills


* Excellent communication and interpersonal abilities


* Ability to work independently and effectively as part of a team

Schedule


* Part-Time


* Flexible schedule with weekday, weekend, holiday, and evening availability as needed
