Job Code: SST012/Social Services Tech 3 Shift: Full-Time (40 hours per week on average) Job Hours: 8:30 PM - 5:00 PM, Monday - Friday with some evening hours required and occasional Saturdays.

Base Location: Covington, GA / Morrow, GA Division/Department/Program: SA Residential and SAIOP/WINGS Full-time X Part-time PRN Exempt X Nonexempt The Care Coordinator is responsible for ensuring that clients are connected with the services that they need and for collaborating with staff across the agency and with external agency partners.

The Care Coordinator serves as a key administrative and operational support role for both the SA Residential and Women Initiating New Growth & Strength (WINGS) Program .This position provides comprehensive support to the Program Manager and treatment teams across these semi-independent SUD programs.

The Care Coordinator will facilitate care coordination between these two programs, ensuring seamless integration of services within the Residential SUD Continuum of Care.

The role is crucial for the smooth operation of both programs, focusing on client support, administrative tasks, and ensuring high-quality care and service delivery.

Under broad supervision, provide a broad range of social services to patients/clients/consumers and their families.

Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention.

Assists in dealing with personal and social problems.

May perform case management duties.

Duties & Responsibilities: Assists with 24x7 staffing coverage.

Provides initial screening of client needs and assists the client in connecting to needed resources .

Connects clients with community services, and provides follow-up and monitoring to ensure continuity of care among internal providers and between internal and external providers.

Facilitates internal and external collaboration to develop person-centered or family-centered integrated client treatment plans.

Assists clients with making and coordinating appointments, and obtaining support necessary to keep appointments.

Facilitates communication between providers within a schedule that meets outlined timeframes.

Develops and maintains close working relationships with community service providers.

Manage day-to-day administrative functions for the program, including scheduling, documentation, and maintaining accurate records in CareLogic.

Ensure that all client intake paperwork, consent forms, and medical records are completed accurately and stored according to confidentiality regulations.

Handle program-related communications, including updating clients on program schedules, resources, and events.

Coordinate logistics for community meetings, support groups, and client activities, ensuring that they align with the program's goals and client needs.
