Position: Part-Time / On-Call
Available Shifts: AM (Day) & PM (Evening)
Pay Rate: $39.00 - $40.00 per hour

Pacific Coast Post Acute is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our clinical team on a Part-Time / On-Call basis.

Key Responsibilities


* Provide direct nursing care in accordance with physician orders, care plans, and facility policies.


* Administer medications, treatments, and nursing procedures safely and accurately.


* Monitor residents' conditions and promptly communicate significant changes to the healthcare team.


* Complete accurate and timely documentation of resident care and clinical observations.


* Collaborate with physicians, therapists, nurses, and interdisciplinary team members to promote positive resident outcomes.


* Ensure resident comfort, dignity, safety, and well-being at all times.


* Maintain compliance with facility policies, infection control standards, and state and federal regulations.

Qualifications


* Current and active California Licensed Vocational Nurse (LVN) license.


* Minimum of 2 years of Skilled Nursing Facility (SNF) experience preferred.


* Current BLS/CPR certification preferred.


* Strong communication, organizational, and critical-thinking skills.


* Ability to work effectively in a fast-paced healthcare environment.
