Position Summary

We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team.

Working under the direction of a licensed Physical Therapist, the PTA will provide skilled therapy services that help residents improve mobility, strength, function, and independence while enhancing their overall quality of life.
Key Responsibilities


* Provide physical therapy treatments as directed by the supervising Physical Therapist


* Implement individualized treatment plans according to physician orders and therapy goals


* Monitor resident progress and communicate updates to the Physical Therapist


* Document treatment sessions, resident responses, and progress notes accurately and timely


* Collaborate with nursing staff and interdisciplinary team members to support positive resident outcomes


* Assist with discharge planning and caregiver education


* Participate in care conferences and team meetings as needed


* Maintain a safe, clean, and organized therapy environment


* Ensure compliance with all state, federal, and facility regulations


* Support residents in achieving their highest level of function and independence

Qualifications
Education & Experience


* Graduate of an accredited Physical Therapist Assistant program


* Experience in skilled nursing, rehabilitation, or long-term care preferred


* Experience with PointClickCare (PCC) and Casamba preferred


* Strong communication, clinical, and documentation skills

Licenses & Certifications


* Current South Carolina Physical Therapist Assistant (PTA) license in good standing


* Must maintain all required continuing education requirements


* Must remain in good standing with the State Board
