Position Summary

As a Licensed Practical Nurse (LPN), you will provide direct nursing care, administer medications, and support residents' overall health and well-being while working closely with the interdisciplinary care team.

Key Responsibilities


* Provide direct patient care in accordance with individualized care plans


* Administer medications and treatments as prescribed


* Monitor resident conditions and report changes to the clinical team


* Supervise and support CNAs to ensure high-quality care delivery


* Maintain accurate and timely clinical documentation


* Promote a safe, clean, and compassionate environment

Qualifications


* Active, unencumbered LPN license in South Carolina (required)


* Current CPR certification required


* Must maintain all required continuing education and remain in good standing with the State Board of Nursing


* Experience with PointClickCare (PCC) preferred


* Strong communication, teamwork, and clinical skills


* Compassion for an underserved population
