90% FTE, day shift, Monday - Friday, 8:00am - 5:00pm with one day off every other week.

You will be working at the Kidney Clinic in Fitchburg, WI and Eastpark Medical Center in Madison, WI.

We are seeking a Registered Nurse (RN) to:


* Provide evidence based, high quality nursing care for patients via telephone and MyChart.


* Assist with care coordination, patient education, facilitate prior authorization and other duties related to support the Polycystic Kidney Disease (PKD) Center of Excellence.


* Work in a fast-paced team-oriented environment, managing a wide variety of tasks including new patient referrals, triage, and assisting with transition to dialysis.


* Utilize critical thinking skills every day.


* Educate our Chronic Kidney Disease patients to better understand their complicated disease process and how to delay disease progression.


* Work as part of a multidisciplinary team for consistent and positive patient outcomes.


* Determine patient care needs by use of the Nursing Process as a framework for assessment and clinical decision making.

Qualifications


* Graduate of school of nursing Required


* Bachelor's Degree in Nursing (BSN) Preferred

Work Experience


* 6 months of RN experience Required


* Relevant RN experience Preferred

Licenses & Certifications


* Registration as a registered nurse in the state where employed or licensure in a state in the licensure compact Upon Hire Required


* CPR/BLS Certification Upon Hire Required


* Applicable clinical certification Preferred
