General Purpose

The Physical Therapist evaluates residents, develops individualized treatment plans, and provides therapeutic interventions designed to improve mobility, strength, balance, endurance, and functional independence.
Essential Duties


* Evaluate residents and determine physical therapy needs and rehabilitation potential


* Develop, implement, and modify individualized treatment plans


* Provide treatment focused on gait training, transfers, balance, strengthening, mobility, and pain management


* Communicate with physicians, nursing staff, residents, and families regarding progress and goals


* Participate in care plan meetings and interdisciplinary team conferences


* Complete timely, accurate, and compliant documentation


* Educate residents and caregivers on exercises, mobility safety, and fall prevention


* Recommend assistive devices and durable medical equipment such as walkers, canes, and wheelchairs


* Supervise PTAs and therapy aides as appropriate


* Maintain a safe treatment environment and follow infection control procedures

Qualifications

Education:


* Doctorate or Master's Degree in Physical Therapy from an accredited program required

License:


* Current California Physical Therapist license in good standing

Preferred Skills:


* Experience with Casamba and PointClickCare


* Strong communication and interpersonal skills


* Excellent clinical judgment and problem-solving abilities


* Passion for patient-centered care and functional outcomes
