Resident Care Manager (RCM) - LPN or RN

What You'll Be Doing

In this role, you'll be at the heart of our care team—supporting residents and helping staff succeed every day.


* Partnering with physicians and the care team to create and carry out personalized care plans


* Supporting and mentoring LPNs and CNAs—answering questions, offering guidance, and helping them grow


* Overseeing admissions, discharges, and transfers to keep things running smoothly


* Keeping a close eye on residents' needs and jumping in when something changes


* Making sure documentation and care plans stay accurate and up to date


* Participating in care conferences and collaborating with the interdisciplinary team


* Helping investigate and follow up on incidents to ensure resident safety


* Supporting infection control efforts and staff education


* Being part of leadership meetings and helping shape quality care initiatives


* Sharing in on-call responsibilities and supporting the team when needed

What You Bring


* Active LPN or RN license in good standing (required)


* Leadership or supervisory experience is a big plus


* A calm, solutions-focused approach to challenges


* Strong communication and organization skills


* A genuine passion for resident care and team support
