COMMUNITY HEALTH WORKER/PROMOTORE I/II
Riverside University Health System - Public Health is seeking Community Health Worker I/II candidates to support the Cardiovascular Health Innovations (CVHI) and Women, Infants, and Children (WIC) programs.
The CVHI and WIC programs provide essential services that support the health and well-being of Riverside County residents by connecting individuals and families to health education, resources, and community services.
The CVHI program focuses on supporting adults 18-65 and older diagnosed with hypertension or high blood cholesterol in Riverside County communities.
The WIC program serves pregnant and postpartum women and children ages 0-5, offering nutrition education and family wellness support.
Two positions are available: one supporting the CVHI program located in Hemet and one supporting the WIC program located in Banning.
Schedule: 9/80 or 5/40.
Positions may require one evening shift per month (until 7:00 PM) and one Saturday clinic shift on a rotating basis.
Hourly Salary Range:
• Community Health Worker I : $19.63-$26.16
• Community Health Worker II : $20.71-$27.60

• Provide health education, instruction, and information in a manner that clients can easily understand and address barriers to physical and mental health services.
• Assist clients in navigating the healthcare system and public health programs by phone and in person to help clients successfully participate in their medical and/or behavioral care.
• Connect program participants to community resources necessary to achieve healthy outcomes; address healthcare barriers and identify and address health-related social needs; make closed-loop referrals to local human service systems to support clients address their needs including but not limited to housing, food, personal safety, transportation, childcare, energy assistance, income assistance, and education.
