FHW Memory Care is a 26-bed secure memory care assisted living community that provides specialized
care for individuals living with Alzheimer’s disease and other forms of dementia.

We are looking for a compassionate Nurse Manager who brings strong clinic skills and thoughtful
leadership to help shape the day-to-day experience for both residents and staff.

In this role, you will provide clinical oversight,
support regulatory compliance, and share on-call with the community administrator to ensure
continuity of care and team support.
Required Qualification
 Graduate of an accredited school of nursing
 Current Colorado RN or LPN license in good standing (RN preferred)
 Current CPR certification
 Minimum one year of experience in a leadership role such as Nurse Manager, Resident Care
Coordinator, or similar in memory care, long-term care, or assisted living.
Position Summary
The Nurse Manager is responsible for leading and coordinating clinical operations in a secure memory
care assisted living environment.

This includes oversight of resident care, regulatory compliance, team
training, medication management, and support for dementia best practice.

The Nurse Manager ensures
consistent, resident centered care while fostering a safe and respectful workplace culture.
Essential Functions
1.

Oversees and coordinates all nursing and direct care services to ensure high-quality, resident-
centered care within the memory care setting.
2.

Completes and tracks required resident assessments, including admission, quarterly, and
assessments for acute changes in condition.
3.

Supports development and timely updates of individualized care plans, including revisions following
significant changes in accordance with CDPHE regulations.

 4.

Participates in admission, transfer, and discharge decision-making to ensure residents meet secure
memory care criteria and clinical safety standards.
