LPN
Job Summary:
The Licensed Practical Nurse (LPN) is responsible for providing direct patient care and supporting the nursing team in a Skilled Nursing Facility (SNF).
The LPN will deliver compassionate care to residents, assist with daily living activities, administer medications, monitor vital signs, and collaborate with the healthcare team to ensure high-quality care.
This role is essential in maintaining a safe, comfortable, and caring environment for residents, while ensuring compliance with regulatory requirements.
Key Responsibilities:
* Resident Care
* Provide direct, hands-on nursing care to residents, including administering medications, dressing changes, and treatments as prescribed by physicians.
+ Monitor and document residents' vital signs (blood pressure, pulse, temperature, etc.), noting any changes in condition and reporting to the Charge Nurse or Director of Nursing (DON).
+ Assist residents with daily activities such as bathing, dressing, grooming, and eating, as needed.
+ Observe and report any physical, mental, or emotional changes in residents' conditions to the nursing supervisor.
+ Help implement individualized care plans in collaboration with the nursing and medical teams.
+ Assist with admission, transfer, and discharge procedures for residents, ensuring accurate documentation.
+ Medication Administration & Documentation
+ Administer medications as prescribed by physicians and in accordance with facility protocols.
+ Maintain accurate and timely documentation of medication administration and any changes in the residents' conditions.
+ Ensure proper storage and handling of medications, including controlled substances, following all regulatory guidelines.
+ Collaboration & Communication
+ Communicate effectively with residents, families, and other healthcare providers to ensure coordinated care.
+ Act as a liaison between the nursing team and other departments, ensuring that all aspects of care are being met.
+ Report changes in residents' health conditions to the Charge Nurse and assist in providing follow-up care as needed.
+ Compliance & Quality Care
+ Follow all facility policies, state regulations, and federal guidelines to ensure compliance with healthcare standards.
+ Participate in care planning meetings and assist in the development of care plans to improve residents' outcomes.
+ Participate in facility audits, quality improvement initiatives, and resident satisfaction surveys.
+ Training & Professional Development
+ Maintain current licensure and stay up to date with best practices in nursing care through continuing education and training opportunities.
