Patient Services Coordinator - CHC-Cardiology (BILINGUAL SPANISH)

ABOUT THE POSITION

The County of Riverside's Department of Riverside University Health System - Community Health Center seeks a Patient Services Coordinator to join their CHC-Cardiology team.

Under minimal supervision, the Patient Services Coordinator will perform a variety of administrative and clerical duties in support of the Riverside University Health System (RUHS) Managed Care or Care Management program; they will also be responsible for submitting OR Case requests for Cath-lab, RUHS, and LLU (along with lab orders), providing pre-op instructions, coordinating pre-ops for surgeries between patient, Physician, and OR, communicating between all Insurances - IEHP, MOLINA, Blue Cross PPO/HMO, coordinating procedures with LLUM Cath-Lab- (PCI, CABG, BiV ICD), coordinating with diagnostics for the following (Cardioversions.

TEE, TTE, Lexiscan), assisting the HF team with hospital D/C appt and referrals, assisting patients with LLUM appointments such as: EP, TAVR, and CT SURGERY, assisting the pharmacist and coordinating (submitting referrals) for the infusion clinic, assisting the clinic with scheduling and rescheduling patients, completing SPEC notes for Cardiology and inbasket messages, submitting referrals through EPIC, and other duties as they apply to the Cardiology Clinic ; as well as, performing others duties as needed within the scope of the classification as required.

This position is located in Moreno Valley.

Bilingual candidates with previous professional experience with EPIC are highly desired.

The schedule for this position is a 5/40 schedule, from 8:30am - 5:00pm.

EXAMPLES OF ESSENTIAL DUTIES
•Assist physicians and other healthcare providers by providing care coordination to ensure patients receive optimal care; interact with Primary Care Providers, Specialty Providers, and other healthcare staff to coordinate care for patients.
•Liaison between healthcare staff and patients regarding maintenance of care and treatment; work closely with Providers to ensure continuity of care and treatment; facilitate communication among patients, physicians, nurses, and other healthcare staff regarding their care, including making appointments, treatment authorizations and resource needs.
•May assist with patient referrals to appropriate community resources; may provide patient education.
•Daily management of referrals which includes, input of data into manage care system, verify member eligibility, provider participation, and tracking of pending referrals.
•Authorize Treatment Authorization Requests (TAR) for the County of Riverside Medically Indigent Services Program (MISP); coordinate Durable Medical Equipment (DME) for all Managed Care Health Plans, public and private, coordinating quotes and equipment availability.
