Responsibilities


* Evaluate and develop individualized treatment plans


* Provide skilled physical therapy services to short-term rehab and long-term care residents


* Collaborate with physicians, nursing staff, and therapy team members


* Monitor progress and update treatment plans as needed


* Educate patients and families on rehabilitation goals and techniques

Qualifications


* Current California Physical Therapist License


* Strong communication and documentation skills


* Skilled nursing experience preferred but not required


* Passion for patient-centered care
