Hayward Gardens Post Acute is seeking a compassionate, motivated, and dedicated Physical Therapist Assistant (PTA) to join our in-house rehabilitation team!

We offer flexible scheduling opportunities, including Full-Time, Part-Time, and PRN positions.

Under the direction and supervision of a licensed Physical Therapist, you will provide hands-on therapy services to help residents improve mobility, strength, balance, coordination, and overall functional independence.

What You'll Do


* Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.


* Assist residents with therapeutic exercises and activities to improve strength, mobility, balance, coordination, and endurance.


* Help residents with transfers, ambulation, positioning, and functional mobility.


* Monitor resident responses to treatment and communicate progress or concerns to the supervising Physical Therapist.


* Document therapy treatments and resident progress accurately and in a timely manner.


* Educate residents and caregivers on exercise programs, mobility techniques, safety strategies, and fall prevention.


* Assist with recommendations and training related to adaptive and mobility equipment.


* Collaborate with nursing staff, physicians, residents, families, and interdisciplinary team members.


* Participate in care conferences and rehabilitation meetings as needed.


* Maintain a safe, clean, and organized therapy environment.


* Follow infection control procedures and facility safety policies.

Qualifications


* Current and active California Physical Therapist Assistant (PTA) license in good standing.


* Graduate of an accredited Physical Therapist Assistant program.


* New graduates are encouraged to apply!


* Skilled nursing or post-acute rehabilitation experience preferred, but not required.


* Strong communication and interpersonal skills.


* Compassionate, professional, and resident-centered approach to care.


* Ability to work effectively as part of an interdisciplinary team.


* Strong organizational skills and attention to detail.
