Full time schedule / On-site Position

Ridgway Post Acute is seeking a caring and organized Social Services Assistant to support our residents and families.

This is a great opportunity for someone who enjoys helping others and wants to grow in the healthcare field while making a real difference in residents' lives.

You'll work closely with our Social Services Director and interdisciplinary team to help residents access resources, coordinate services, and maintain the highest level of well-being.

What You'll Do


* Assist the Social Services Director with social service programs and resident support


* Help coordinate appointments, services, and community resources


* Support resident and family communication


* Assist with discharge planning and care coordination


* Maintain social service documentation and reports

What We're Looking For


* Associate's or Bachelor's Degree preferred (not required)


* Healthcare experience preferred


* Strong communication and organizational skills


* Passion for helping seniors and supporting families
