What Does The Nurse Practitioner Do Every Day? Collaborates with the IDT in assuring appropriate evaluation and palliative treatment of terminal disease.Consult with and assist in the IDTs development of patient/family plan of care to assure quality care is provided.Educate the patient/family regarding disease process, self-care techniques, end-of-life care.Provide training to other clinical staff as needed.Perform and document visits to patients/family based on a plan of care.Actively participate in the interdisciplinary team.Provide clinical assessment results to hospice physicians after making face-to-face encounters with patients.

Background and Education You Need to Be Successful: Graduate of an accredited School of Nursing programCurrent State license as a Registered Nurse and as an Advanced Practice NurseCurrent Drivers License in the state in which the role is located2 years of experience as an APN, in a clinical care setting, with background with a hospice agency required.Comprehensive knowledge of pharmacology, palliative care, and end of life care.Exceptional communication and interpersonal skillsProof of Hepatitis consent/declination.
