Family Service Coordinator II

The full-time (40 hour/week) Family Service Coordinator II coordinates program services provided by the early intervention team, supports and advocates for families and children, and ensures compliance by monitoring written documentation.

Responsibilities: Makesthe initial family contact and gathers information to identify family concerns, priorities, and resources.Educates families of their rights and responsibilities.Completes Concerns, Priorities, and Resources (CPR) CPR pages.Schedules evaluations and assists the evaluation team in determining program eligibility.Ensures compliance with the Individualized Family Service Plan (IFSP).Coordinates educational, community, and social services resources for families based on the IFSP.Assists in identifying social service needs for families and refers them to the program director for help in accessing community resources when appropriate.Coordinates upcoming due dates for six-month reviews, annual assessments, and transition reports; and schedules and coordinates regular evaluations, IFSP meetings, transition meetings, and closure meetings.Ensures a seamless transition of children from DDI Vantage to other agencies.Assists in child-find activities.Attends all mandatory Comprehensive System of Personal Development (CSPD) training for early intervention credentialing.Participates in DDI Vantage staff in-service training, attends workshops and conferences as approved.Completes written documentation in a timely manner to ensure program compliance.Attends team meetings as necessary and all mandatory DDI Vantage meetings.Demonstrates professionalism in all aspects of the job.Safeguards the confidentiality of all clients, at all times.Fulfills other duties as assigned.
