* Our Community Resource Liaison Will: Guide the organization to sustainable, ethical, and profitable growth as measured by goals for ADC, budgeting, and financial metrics.Work to ensure meaningful and effective engagement and relationships with communities, families, and patients as measured by community utilization and feedback.Implement marketing and promotional initiatives that are consistent with our core values and help to improve our branding and the publics perception of services offered.Provide educational in-services to Heritage Communities residents and associates on a regular basis.

Become a resource for customers and the community regarding all service lines.

Foster a culture in alignment with the Heritage Core Values as measured by associate feedback and surveys.

Serve as a knowledgeable resource on all OnCare Health programs and services, providing education and guidance to residents, families, and community teams.Document all resident interactions and follow-up in the Customer Relationship Management system.Assist the Health and Wellness Navigators in marketing and community engagement efforts to achieve long-term, sustainable growth.Communicate daily with the Navigator team regarding resident needs and follow-up.Maintain standards of high-quality customer service Participate in department meetings, team meetings, in-services, and other related activities.Maintain confidentiality about all patient personal, financial, and health information including following HIPAA guidelines.

Demonstrate the ability to think critically, plan, organize, and problem solve.Take the initiative to ensure Core Values are followed and patients, families, and communities receive excellent care.Demonstrate attention to detail, flexibility, and remains calm in a fast-paced, sometimes stressful environment.Promote a positive, supportive working environment for all team members.
