SIGN-ON BONUS: $2,000

Full-time new hires are eligible for a $2,000 sign-on bonus, paid in two installments:


* $1,000 (50%) after completion of 90 days of continuous employment


* $1,000 (50%) after completion of 180 days of continuous employment

Bonus payments are contingent upon remaining actively employed and in good standing at the time each payment is due.

About Santa Rosa Post Acute

Our therapy department partners closely with nursing, physicians, and case management to create individualized treatment plans that help residents achieve their highest level of independence and quality of life.

Responsibilities


* Evaluate residents and develop individualized occupational therapy treatment plans


* Provide skilled occupational therapy interventions in accordance with physician orders and established care plans


* Help patients improve performance in activities of daily living (ADLs), self-care tasks, mobility, cognition, and functional independence


* Collaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and families to coordinate care


* Complete timely and accurate documentation in compliance with regulatory and facility requirements


* Participate in interdisciplinary team meetings, care conferences, and discharge planning


* Educate patients, families, and caregivers regarding therapeutic techniques and home programs


* Maintain compliance with all federal, state, and professional standards of practice
