What You'll Do


* Provide high-quality nursing care to residents in accordance with individualized care plans.


* Administer medications and treatments as ordered.


* Monitor residents' conditions and report changes to the RN and medical team.


* Complete accurate and timely clinical documentation.


* Work closely with CNAs, RNs, physicians, and other members of the interdisciplinary team.


* Assist with assessments, care planning, and resident needs.


* Promote resident safety, dignity, comfort, and quality of life.


* Communicate effectively with residents and their families.

What We're Looking For


* Current New York State LPN license.


* Previous nursing experience preferred; skilled nursing experience is a plus.


* Strong clinical, communication, and organizational skills.


* Compassionate and resident-focused approach.


* Ability to work effectively as part of a team.
