Hayward Gardens Post Acute is seeking a compassionate, dependable, and dedicated Licensed Vocational Nurse (LVN) to join our clinical team on a part-time basis.

The LVN will provide quality nursing care to our residents while working collaboratively with the interdisciplinary healthcare team.

Position Responsibilities


* Provide direct nursing care to residents in accordance with physician orders and facility policies.


* Administer medications and treatments safely and accurately.


* Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.


* Complete accurate and timely nursing documentation.


* Assist with care planning and communicate resident needs to the interdisciplinary team.


* Support CNAs and other nursing staff in providing quality resident care.


* Maintain a safe, clean, and comfortable environment for residents.


* Follow all applicable state, federal, and facility regulations.


* Promote resident dignity, independence, and quality of life.

Qualifications


* Current and active California LVN license.


* Current CPR certification.


* Previous skilled nursing or post-acute experience preferred.


* Strong communication and organizational skills.


* Reliable, compassionate, and team-oriented.


* Ability to provide professional and resident-centered care.
