Home-Based Coach (Union) Location: 765 South Buchanan Street, Fremont, OH 43420 Schedule: Monday - Thursday Weekly Hours: 34 hours per week Employment Type: Year-Round Full-Time Pay Rate: $21.34 - $23.47 per hour, based on education Position Overview: Provide home-based services as an adult educator to assigned families, supporting child development, family engagement, health and social services, and school readiness in compliance with Head Start Performance Standards, licensing requirements, and agency policies and procedures.

Experience: At least one year of experience working with children and families and one year of experience training or educating adults.

Experience with word processing, spreadsheets, internet, and database software preferred.

Duties and Responsibilities: Recruit, enroll and maintain required caseloads according to the requirements outlined in the ERSEA plan.

Collate information for health packets and ensure children are up to date with health services according to EPSDT.

Conduct health and vision screenings as needed.

Visit families as assigned and ensure completion of the requirements outlined in Parent Family and Community Engagement (PFCE) and Early Childhood Development and Health Services performance standards.

Ensure appropriate referrals and follow up for identified medical and social service needs of children and families.

Schedule 90-minute in-home visits weekly with children and their families per program requirements; make up cancelled visitsper performance standards and written program procedures.

Plan and facilitate Family Socialization days 2 times per month and assist with other group family events as outlined in written plans.
