The LPN works collaboratively with patients, families, physicians, therapists, and interdisciplinary team members to promote positive patient outcomes and exceptional care experiences.

Essential Responsibilities Provide skilled nursing care in patients' homes according to physician orders and individualized care plansPerform patient assessments and report changes in condition to the supervising RN and physician as appropriateAdminister medications and treatments safely and accuratelyMonitor and document patient progress, responses to treatment, and ongoing care needsProvide patient and family education regarding medications, disease processes, treatments, and self-care techniquesAssist in coordinating services with physicians, therapists, caregivers, and other healthcare professionalsEnsure accurate, timely, and compliant clinical documentationParticipate in care conferences and interdisciplinary team meetingsFollow all agency policies, infection prevention protocols, and regulatory requirementsSupport quality improvement initiatives and pat...
