Peninsula Post Acute is seeking a compassionate, organized, and motivated Admissions Coordinator to join our team.

Pay: $29.00-$30.00 per hour
Schedule: Friday - Sunday

General Purpose

The Admissions Coordinator plays a vital role in ensuring a smooth, welcoming, and efficient transition for new residents and their families.

This position serves as a key point of contact throughout the admissions process and works closely with hospitals, referral sources, residents, families, and interdisciplinary teams to coordinate timely and compassionate admissions.

The ideal candidate is highly organized, detail-oriented, professional, and comfortable working in a fast-paced healthcare environment.

Essential Duties and Responsibilities


* Serve as the primary point of contact for prospective residents, families, hospitals, physicians, case managers, and referral sources.


* Coordinate and schedule resident admissions, including pre-admission assessments and required documentation.


* Maintain accurate and up-to-date records in accordance with federal, state, and facility regulations.


* Assist with insurance verification, authorizations, Medicare/Medicaid processes, and financial eligibility requirements.


* Provide facility tours and communicate information about services, programs, and the admission process to prospective residents and families.


* Collaborate with nursing, social services, business office, and administrative staff to ensure a seamless admission experience.


* Prepare, organize, and distribute admission packets and required documentation.


* Follow up with hospitals, physicians, case managers, and other referral sources to ensure timely and complete transfer of information.


* Communicate professionally and compassionately with residents and families throughout the admissions process.


* Support facility marketing, referral development, and community outreach efforts as needed.


* Maintain confidentiality of resident and facility information at all times.


* Perform other duties as assigned.

Qualifications

Education and Experience


* Prior experience in admissions, healthcare, skilled nursing, long-term care, or a related setting is strongly preferred.


* Knowledge of Medicare, Medicaid, insurance verification, and authorization processes preferred.


* Experience working with hospitals, referral sources, case managers, or healthcare providers is a plus.
