What You'll Do


* Coordinate and complete MDS assessments in accordance with state and federal regulations.


* Ensure accurate and timely submission of MDS documentation.


* Collaborate with interdisciplinary teams to develop and update resident care plans.


* Monitor clinical documentation to support reimbursement and care quality.


* Participate in quality assurance and compliance initiatives.


* Serve as a resource for nursing staff regarding MDS and care planning processes.

What We're Looking For


* Current RN or LPN license in good standing.


* Previous MDS experience in a skilled nursing or long-term care setting preferred.


* Strong knowledge of Medicare, Medicaid, and PPS processes.


* Excellent organizational and communication skills.


* Ability to work independently while collaborating effectively with team members.


* Commitment to providing exceptional resident-centered care.
