Position Summary

We are seeking a compassionate and skilled Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis.

In this role, you will evaluate and treat residents, develop individualized treatment plans, and work closely with residents, families, physicians, and interdisciplinary team members to maximize independence and quality of life.
Key Responsibilities


* Evaluate residents and develop individualized occupational therapy treatment plans


* Provide skilled occupational therapy services in accordance with physician orders and resident goals


* Monitor resident progress and adjust treatment plans as needed


* Collaborate with physicians, nursing staff, residents, families, and interdisciplinary team members


* Participate in care plan meetings, rehabilitation conferences, and discharge planning activities


* Complete accurate and timely documentation in accordance with facility, state, federal, and payer requirements


* Educate residents, families, and caregivers on treatment plans, adaptive techniques, and maintenance programs


* Recommend adaptive equipment and interventions to improve resident function and safety


* Conduct in-service training and education for facility staff as needed


* Maintain compliance with all facility policies, state practice acts, and regulatory requirements


* Promote resident safety, independence, and overall well-being

Qualifications
Education & Experience


* Bachelor's degree in Occupational Therapy required


* Master's or Doctorate degree in Occupational Therapy preferred


* Experience in skilled nursing, rehabilitation, or long-term care preferred


* Experience with PointClickCare (PCC) and Casamba preferred


* Strong clinical, communication, and documentation skills

Licenses & Certifications


* Current Occupational Therapist license in South Carolina and in good standing
