* Pay Rate: $36.00-$40.00 per hour

Responsibilities


* Provide compassionate, high-quality nursing care to residents in accordance with individualized care plans.


* Administer medications and treatments as ordered and in accordance with facility policies.


* Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.


* Perform nursing assessments and document resident care accurately and timely.


* Assist with wound care, treatments, and other nursing procedures as directed.


* Monitor vital signs and other clinical indicators as appropriate.


* Communicate effectively with residents, families, physicians, and members of the interdisciplinary care team.


* Assist with admissions, transfers, and discharges as needed.


* Maintain accurate and complete nursing documentation.


* Follow all applicable federal, state, and facility regulations, policies, and procedures.


* Promote resident safety, dignity, independence, and quality of life.

Qualifications


* Current and active California LVN license in good standing.


* Current CPR certification preferred/required per facility policy.


* Previous skilled nursing or post-acute care experience preferred.


* Strong clinical assessment, communication, and organizational skills.


* Compassionate, dependable, and resident-focused.


* Ability to work effectively as part of an interdisciplinary healthcare team.


* Must be able to commit to 24-32 hours per week.
