Registered Nurse (RN) Case Manager

RN Case Manager Full-Time The RN Case Manager is responsible for providing skilled nursing care and coordinating services for patients receiving home health care services.

The RN Case Manager works closely with physicians, patients, caregivers, and the interdisciplinary team to ensure quality care and positive patient outcomes.

Essential Duties & Responsibilities Perform patient assessments and develop individualized care plansComplete admissions, routine visits, recertifications, and discharge visitsAdminister skilled nursing care according to physician ordersEducate patients and families regarding disease management and treatment plansCoordinate care with physicians and interdisciplinary team membersEnsure timely and accurate clinical documentationMonitor patient progress and communicate changes appropriatelyParticipate in care conferences and quality improvement activitiesMaintain compliance with agency policies and regulatory requirements Qualifications Current RN license in applicable stateHome Health experience preferredStrong clinical assessment and documentation skillsExcellent communication and time management skillsCurrent CPR certificationValid drivers license and reliable transportation
