Social Worker - IL

The Social Worker IL serves as a key resource for Independent Living residents throughout their experience at The Admiral at the Lake.

Responsibilities include coordinating the pre-admission process, conducting psychosocial and cognitive assessments, identifying appropriate services and resources, and helping residents navigate healthcare, financial, legal, mental health, and community resources.

This position provides ongoing support to residents and families during changes in health, illness, disability, hospitalization, discharge, and transitions to higher levels of care.

The Social Worker coordinates with healthcare providers, facilitates wellness and at-risk resident meetings, supports discharge planning, and helps arrange home care and other community-based services when needed.

The role also serves as a liaison between residents, families, internal departments, and outside providers while maintaining appropriate documentation in the electronic medical record (EMR).

The Social Worker contributes to campus wellness initiatives, community partnerships, policy development, student supervision, and programs that support the physical, emotional, social, and mental well-being of older adults.
