๐ Responsibilities:
๐น Evaluate residents with speech, language, cognitive, communication, and swallowing disorders
๐น Develop individualized treatment plans based on resident needs and goals
๐น Provide skilled speech therapy services in accordance with physician orders and established care plans
๐น Educate residents, families, and caregivers on therapy goals and treatment techniques
๐น Collaborate with interdisciplinary team members to ensure optimal patient outcomes
๐น Complete timely and accurate documentation in compliance with regulatory requirements
๐น Participate in care plan meetings and discharge planning as needed
๐น Maintain compliance with all federal, state, and facility guidelines
๐ Qualifications:
โ๏ธ Master's Degree in Speech-Language Pathology
โ๏ธ Current Speech-Language Pathologist License (State Requirements)
โ๏ธ Certificate of Clinical Competence (CCC-SLP) preferred
โ๏ธ Experience in skilled nursing or rehabilitation settings preferred, but not required
โ๏ธ Strong communication and interpersonal skills
โ๏ธ Passion for helping others achieve their highest level of independence
