Veteran Services Case Manager (Family Advocate III)
Veteran Services Case Manager (Family Advocate III) Office Location: 812 North College Drive, Bowling Green, OH 43402 Service Area: This position primarily serves veterans in Wood County and Lucas County.
Regular travel throughout these counties is required to meet with participants, conduct home visits, engage with landlords, and coordinate services.
Occasional travel to other areas within GLCAP's service region may be required based on program or organizational needs.
Schedule: Monday - Thursday Weekly Hours: 34 hours per week Employment Type: Year-Round Full-Time Pay Rate: $23.47 - $26.00 per hour, based on education and experience Position Overview:Provide case management and supportive services to individuals and families experiencing housing instability or other barriers to self-sufficiency.
Assess participant needs, coordinate community resources, and build collaborative relationships with service providers and property owners to promote housing stability and long-term success.

Experience: At least three years of experience providing case management or working with at-risk youth, adults, or families.
Duties and Responsibilities: Initiate and maintain relationships with other agency departments, area social service providers, and property owners to assist participants and families in meeting their needs.
Recruit, screen, assess, establish goals, and make referrals related to employment, housing, health care including substance abuse and/or alcohol abuse, economic self-sufficiency, and interpersonal relationships to assist participants and their families meet their basic needs.
