About the Role

Midtown Oaks Post Acute is hiring a Physical Therapist (PT) to join our rehabilitation team in Sacramento, CA.

We welcome both experienced clinicians and new graduates.

In this role, you will provide skilled physical therapy services to residents recovering from illness, injury, and surgery in a skilled nursing and post-acute setting.

Physical Therapist Responsibilities


* Evaluate residents and develop individualized treatment plans


* Deliver evidence-based physical therapy services in a skilled nursing and post-acute setting


* Establish functional goals to help patients regain mobility and independence


* Complete evaluations, progress notes, discharge summaries, and required documentation


* Collaborate with physicians, nursing staff, families, and interdisciplinary team members


* Educate patients and caregivers on treatment plans and home exercise programs


* Support positive patient outcomes and satisfaction

Physical Therapist Qualifications


* Active California Physical Therapist (PT) license, or eligibility to obtain licensure


* New graduates encouraged to apply


* Experience in skilled nursing, rehabilitation, or post-acute care preferred but not required


* Strong clinical reasoning and communication skills


* Strong documentation and time-management skills


* Commitment to patient-centered, compassionate care
