RN Clinical Liaison

Hospice of Orange & Sullivan Counties (HOSC) is seeking an experienced, compassionate Registered Nurse Clinical Liaison to join our Patient Access team.

This is an exciting opportunity for an RN who enjoys combining clinical expertise with relationship-building, education, and care coordination to improve access to hospice services throughout our community.

As an RN Clinical Liaison, you will serve as a trusted clinical resource for hospitals, healthcare providers, patients, and families while partnering with our interdisciplinary team to facilitate timely, seamless transitions to hospice care.

You'll play a key role in expanding access to compassionate end-of-life care by educating referral sources, conducting hospice evaluations, supporting admissions, and strengthening community partnerships.
