Piedmont Post Acute is an 88-bed skilled nursing facility located in Piedmont, South Carolina—just 20 minutes from Greenville.

Our smaller size creates a quiet, close-knit, family-oriented environment where we truly care about our residents and each other.

We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehab team to help residents reach their highest level of independence.

Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and overall function.

You'll play a key role in helping residents regain independence and improve quality of life.

Key Responsibilities


* Perform patient evaluations and develop individualized treatment plans


* Provide therapeutic interventions to improve mobility, strength, and balance


* Monitor progress and adjust plans as needed


* Maintain accurate and timely documentation


* Collaborate with interdisciplinary team members


* Educate residents and families on treatment plans and goals

Qualifications


* Active, unencumbered Physical Therapist (PT) license in South Carolina (required)


* Degree in Physical Therapy (Bachelor's, Master's, or Doctorate)


* Experience in long-term care or rehab setting preferred


* Familiarity with PointClickCare (PCC) and Casamba preferred


* Strong communication and clinical skills


* Passion for improving resident outcomes
