Position Details

Position: Part-Time LVN
Location: Tice Valley Post Acute - Walnut Creek, CA
Pay Rate: $35.00 per hour

Key Responsibilities

Clinical Care


* Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs


* Deliver compassionate, resident-centered care focused on comfort, dignity, and safety

Assessment & Monitoring


* Observe, assess, and document changes in resident condition


* Communicate concerns promptly to the RN Supervisor and healthcare providers

Care Planning


* Assist with the development and implementation of individualized care plans


* Support resident recovery goals and quality-of-life initiatives

Leadership & Team Support


* Supervise and support Certified Nursing Assistants (CNAs)


* Collaborate with interdisciplinary team members to ensure continuity of care

Documentation & Compliance


* Maintain accurate clinical documentation and medical records


* Ensure compliance with all state, federal, and facility regulations and standards

Qualifications

✔ Current and active California LVN license required
✔ Current BLS/CPR certification required
✔ Previous skilled nursing or post-acute care experience preferred
✔ Strong clinical judgment and communication skills
✔ Compassionate, dependable, and team-oriented approach to resident care
