Position Summary

As a Wound Care Nurse (LPN), you will be responsible for providing specialized skin and wound care to residents under the direction of the physician, Director of Nursing, and/or Medical Director.

You will play a key role in promoting healing, preventing complications, and improving overall resident outcomes.

Key Responsibilities


* Provide direct wound care and skin treatments in accordance with physician orders


* Assess, monitor, and document wound progress and treatment effectiveness


* Assist in developing and modifying individualized care plans


* Collaborate with the interdisciplinary team to ensure optimal outcomes


* Educate staff on proper wound care techniques and prevention strategies


* Maintain accurate and compliant clinical documentation


* Ensure adherence to all regulatory, safety, and infection control standards

Qualifications


* Active, unencumbered LPN license in South Carolina


* Graduate of an accredited Practical Nursing program


* Current CPR certification required


* Ability to work 12-hour shifts


* Experience with PointClickCare (PCC) preferred


* Prior wound care experience strongly preferred


* Strong clinical judgment and attention to detail


* Passion for delivering compassionate, resident-centered care
