The Skilled Service Coordinator is responsible for managing and facilitating the admission process for all new skilled residents, ensuring a seamless transition into the nursing home.

This role serves as a key liaison between residents, families and staff to promote a comfortable, supportive and high-quality care experience.

The coordinator proactively addresses concerns, resolves issues and ensures resident satisfaction throughout their stay.

Full-Time

Shift: Monday- Friday 8:00am-5:00pm

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Admission Coordination
• Oversee and coordinate all aspects of new resident admissions.
• Ensure a smooth, welcoming and efficient onboarding experience.
• Collaborate with clinical and administrative teams to prepare for arrivals.

• Resident & Family Support
• Serve as the primary point of contact for residents and their families.
• Maintain resident communication to ensure needs, expectations and concerns are addressed
• Provide guidance and support throughout the residents' stay.

• Care Experience Oversight
• Monitor resident satisfaction and comfort levels
• Conduct check-ins with residents and families to assess well-being and service quality.
• Advocate for residents' needs with interdisciplinary teams.

• Interdepartmental Collaboration
• Work closely with nursing, therapy, social services and administrative staff
• Ensure continuity of care and effective communication across departments.
