Santa Rosa Post Acute is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our growing therapy team.

What You'll Do:


* Evaluate patients within 24 hours (or 1 business day) of referral


* Develop and implement individualized treatment plans


* Provide therapy services in accordance with physician orders


* Supervise Speech-Language Pathologist Assistants as applicable


* Collaborate with interdisciplinary teams on patient progress and care plans


* Participate in care conferences, utilization review, and rehab meetings


* Complete accurate and timely documentation (evaluations, daily notes, progress reports)


* Educate patients, families, and staff on therapy plans and discharge needs


* Assist with discharge planning and caregiver training


* Provide in-service training and support ongoing staff development

What We're Looking For:


* Master's or Doctoral degree in Speech-Language Pathology


* Current California SLP license (or eligibility) in good standing


* Strong communication and organizational skills


* Ability to thrive in a fast-paced, team-driven environment


* Experience in skilled nursing or post-acute care preferred

Additional Requirements:


* Maintain compliance with state practice acts and facility policies


* Meet continuing education requirements


* Proficiency with EMR systems (e.g.,Net Health, PointClickCare, Casamba) is a plus
