Physical Therapist (PT) - PRN

Position Summary

Tice Valley Post Acute is seeking a skilled and compassionate PRN Physical Therapist to join our rehabilitation team.

The ideal candidate will provide high-quality, patient-centered care to help residents regain mobility, improve functional independence, and enhance overall quality of life.
Key Responsibilities


* Evaluate residents and develop individualized treatment plans based on clinical findings


* Implement therapeutic interventions to improve strength, mobility, balance, and coordination


* Monitor patient progress and adjust treatment plans as needed


* Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists


* Educate patients and families on treatment plans, exercises, and safety strategies


* Maintain accurate and timely documentation in compliance with regulatory standards


* Ensure adherence to all facility policies and professional guidelines

Qualifications


* Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program


* Current California Physical Therapist license (in good standing)


* Strong clinical assessment and treatment skills


* Excellent communication and interpersonal abilities


* Experience in skilled nursing or post-acute care preferred, but not required
