What You'll Do
As a Resident Care Manager, you'll play a key leadership role in ensuring residents receive exceptional care while supporting and guiding the clinical team.

Responsibilities include:


* Coordinating and overseeing daily nursing care in collaboration with physicians and the interdisciplinary team


* Leading and supporting licensed nurses and CNAs through coaching, guidance, and daily oversight


* Assisting with admissions, transfers, and discharges while ensuring accurate documentation


* Completing and managing MDS assessments and related clinical documentation


* Conducting care conferences and collaborating with residents and families to create individualized care plans


* Monitoring resident conditions and ensuring care plans are updated and followed appropriately


* Promoting a positive and supportive environment for residents and staff

What We're Looking For


* Current Registered Nurse (RN) or (LPN) license required


* Strong leadership and team-building skills


* Experience in skilled nursing or long-term care preferred


* Knowledge of care planning, MDS, and regulatory compliance


* Passion for providing high-quality resident-centered care
