Full-time or part-time opportunities available

Key Responsibilities


* Provide physical therapy treatments under the direction of the Physical Therapist


* Follow individualized treatment plans and physician orders


* Document daily treatment notes and weekly progress notes in accordance with PT Board guidelines


* Communicate patient progress with the therapy and interdisciplinary care team


* Participate in care plan meetings and rehabilitation conferences as needed


* Educate patients, families, and nursing staff on therapy programs and discharge planning


* Assist with training restorative nursing aides when appropriate


* Maintain therapy equipment and ensure the treatment area is clean and organized

Qualifications


* Active California Physical Therapist Assistant (PTA) license


* License must be in good standing


* Experience in skilled nursing, post-acute, or rehabilitation settings preferred


* Familiarity with Casamba, PointClickCare, or similar EMR systems is a plus


* Strong communication and teamwork skills


* Passion for helping patients regain independence and mobility
