* Position: Full-Time, Part-Time, or PRN (Per-Diem)


* Location: City View Post Acute — San Francisco, CA


* Rate: Starting at $ 55.00-$ 65.00 per hour (Depending on Experience)


* Sign-On/Retention Bonus: $5,000.00

Your Impact & Responsibilities:



* Clinical Evaluation: Evaluate patients, establish individualized treatment plans, and set measurable functional goals.


* Evidence-Based Therapy: Deliver OT interventions to maximize patient independence in activities of daily living (ADLs).


* Precise Documentation: Accurately maintain evaluations, progress notes, and discharge summaries in a timely manner.


* Education & Advocacy: Educate patients, families, and caregivers on adaptive techniques, home programs, and safety strategies.


* Interdisciplinary Teamwork: Collaborate with the rehab and nursing teams to optimize patient outcomes and participate in quality-improvement initiatives.

Qualifications:



* Licensure: Current, valid California Occupational Therapist (OT) license (or eligibility to obtain).
